The purpose of this survey is to profile the level of maturity of an organization's approaches to the business use and management of information technology.  This survey should be completed by an individual familiar with the business strategy for technology use, the roles and work of the IT organization, and the relationship between IT and the rest of the business.

This survey consists of 50 sets of statements - grouped into 10 categories of 5 sets each. For each set of three statements, please choose the statement (1, 2, or 3) that most closely matches the current state of capability or focus in your business and its IT organization. If the capability is in transition, or split between two of the choices, please choose 1.5 or 2.5. If all three seem to apply, please choose the one that comes closest to representing the current emphasis in the business.

Some statements may seem similar. Please consider each statement set in light of its category.

Note that questions 6 - 20 are about governance capabilities, questions 21 - 35 are about the delivery of technology to the business, and questions 36 - 50 are more behind-the-scenes capabilities that enable delivery.
